Video Appointments
Requirements

  1. Check blood pressure on your home blood pressure unit while medications are on board and within two days of your appt.

  2. Send picture of the digital display of your blood pressure to the portal->messages->compose-> attach->send. 

  3. Complete questionnaires in in the portal for all three screenings.

click on the link in the Charm email or text sent to you to start our Zoom video session.
